A few years ago our Local Spiritual Assembly recognized that although we knew the members of our community to see them at Feasts, Holy Days, Assembly meetings etc; we really didn’t know who each other was; or what strengths and areas of contribution each of us could add, so I developed the following questionnaire, which was given to everyone in the community to fill out.  Feel free to use it if you would find it helpful!
